At Janssen, we don't want cost to get in the way of treatment you need. We can help you explore options to lower your out-of-pocket cost for INVOKANA®, INVOKAMET® XR, and INVOKAMET®. No matter what type of insurance you have—or even if you don't have insurance—Janssen CarePath can help find programs that may help you pay for INVOKANA®, INVOKAMET® XR, and INVOKAMET®.
If you have commercial or private health insurance and need help paying for INVOKANA®, INVOKAMET® XR, and INVOKAMET®, the Janssen CarePath Savings Program may be able to help. If you don’t have commercial or private health insurance, you may find help from the programs and resources found on JanssenCarePath.com.
If you use commercial or private health insurance to pay for your medication:
Janssen CarePath Savings Program for INVOKANA®, INVOKAMET® XR, and INVOKAMET®
- Eligible commercial or private patients pay $0 per month for INVOKANA®, INVOKAMET® XR, and INVOKAMET®, subject to monthly program benefit limits. There is no limit to this benefit for your first month of treatment, and then a $200 limit for each month thereafter. There is a $3,000 maximum program benefit per calendar year.
- Not valid for patients using Medicare, Medicaid, or other government-funded programs to pay for their medications. Terms expire at the end of each calendar year and may change. There is no income requirement. See program requirements.
- When you register, you'll receive a Janssen CarePath Savings Program card. Download and print the card, and present the card at your pharmacy along with a valid prescription. Your eligibility to use this card is dependent upon meeting the program requirements each time you present the card when filling your prescription.
If you are using a government-funded healthcare program to pay for your medication or have no insurance coverage and need help paying for INVOKANA®, INVOKAMET® XR, and INVOKAMET®:
Janssen CarePath can provide information about other resources that may be able to help you with your out-of-pocket medication costs including State-Sponsored Programs, Medicare Savings Program, Medicare Part D Extra Help—Low-Income Subsidy, and Independent Foundations.* You may also find help from the programs and resources shown at JanssenCarePath.com.
*Independent co-pay assistance foundations have their own rules for eligibility, which are subject to change. We cannot guarantee a foundation will help you. We can only refer you to foundations that support your disease state. This information is provided as a resource for you. We do not endorse any particular foundation.
If your insurance plan doesn’t approve INVOKANA®, INVOKAMET® XR, or INVOKAMET®:
Sometimes, you may experience problems getting a medication that your doctor has prescribed for you. For example, your insurance plan might not cover that medication because it’s not on the plan’s drug list (formulary), or the cost is higher than you think you should have to pay. In these situations, you and your doctor have the right to ask the insurance plan to make the medication available to you as an exception to its policies, or to explain a coverage denial and reconsider. To help, you can ask your doctor to complete a Letter of Medical Necessity or Exception Letter—and samples of both letters are available to your doctor on JanssenCarePath.com.
